Carolyn Dean is both a medical doctor and naturopathic doctor
who assists individuals with all types of illness at any stage
or severity. She has been in the forefront of health issues
for twenty-eight years.
Dr.

Your Email
You will receive a one-page Intake Form, a one-page Health
Index, and a one page Consultation Agreement. You will
receive these 3 pages in two different formats for easier
access. One set in PDF and one in MS Word. Directions
will be given for you to return the forms by fax or by
mail.
In
a letter from Dr. Dean that comes with these forms you
will be asked to send a detailed health history and a
list of your complaints and questions by email to help
Dr Dean assess your unique condition as accurately as
possible.

Arrange
A Time
Dr. Dean will contact you by email or phone to arrange
for a consultation at your earliest convenience. When
the time is set, you will call Dr. Dean at a number that
she will provide.
Note:
While the preferred appointment method is by email, those
without access to a computer may set up an appointment
by calling 718-885-1104. The fee for Dr. Dean's services
is $200 per consultation hour, which can be paid by credit
card at the time of the appointment.
